Introduction

CCACC Adult Day Healthcare Center (CCACC ADHC) is a community-based program design to meet the needs of functionally and/or cognitively impaired adults through an individual plan of care. These structured comprehensive programs provides a variety of health, social and other related support services in a protective setting during the day.

To enhance the quality and dignity of life for individuals who have disabilities or illness through a supportive and rehabilitative day program, which maximizes independence and therefore enables the individual to remain in his/her home environment.

 

VALUES:

1. The participant is always recognized and respected;

2. Human dignity must be preserved above all, while striving to help each participant to obtain his/her optimal state of living;

3. The needs of the participant must be fulfilled, whether those needs are for health care, social services, or emotional support; and

4. The participant's relationships with their family and community should be supported and maintained.

 

Services And Programs

Door-to-door transportation

Medical care administered by registered nurses(RN) and certified nursing assistants(CNA)

Nutritious meals and snacks

Stimulating and therapeutic activities, exercises and rehabilitation

Friendly social environment

Personalized care and counseling

Transportation to see doctors and prescription services

 

Participation Information

CCACC ADHC is an approved Medicaid Service Provider by the State of Maryland. If a medi-caid recipient is medically certified, all fees will be covered by Medicaid. Although ADHC is open to all participants, the Medicaid coverage is limited to residents of Maryland. Other state residents may participate without Medicaid assistance. Self-pay participants are also welcome.

 

There are three ways to participate our program.

1. Medicaid Recipients: Need to file an application (we could help), go through interview (will be conducted by a county nurse), get approved through InterRAI Assessment,

2. self pay: The fee is $80.00 per day. The services include medical care, transportation (door to door pick up), two meals and snacks, a variety of culture related activities etc. The bus pick up is scheduled for the time between 8:00am and 9:00am and going home between 1:30pm and 1:45pm.

3. Applicants for Montgomery County Adult Day Care Subsidy Program (Senior Care or County Fund)

Eligibility requirements: 1) Montgomery County Resident-adult, 2) Moderate-to-Severe Physical or Cognitive Disability, 3) Asset and Income Eligibility Guidelines--Monthly income below $2,739.00 for an individual; $3,581.00 for a married couple, Assets below $110,000.00 for an individual; $140,000.00 for a married couple (excludes house and car) Once they are admitted and start receiving their allotted funds, they can participate in our program for free twice a week. CCACC ADHC staff will also help them with applications.
